    Introduction

    Governance is the connective tissue that holds a program together. It provides the structure for decision-making, the mechanisms for accountability, and the processes for ensuring that the program remains aligned with organizational strategy. Without effective governance, programs drift. Decisions get delayed because nobody knows who has the authority to make them. Risks go unaddressed because there is no escalation pathway. Performance problems persist because there is no review mechanism to catch them. And strategic misalignment goes undetected because nobody is asking whether the program still supports the organization's goals.

    This chapter covers the design, implementation, and operation of the program governance framework. You will learn how to structure a governance board, define decision rights and escalation pathways, conduct stage gate reviews, monitor performance through KPIs, manage risk at the governance level, maintain compliance, and build a lessons learned repository that actually improves organizational practice. The chapter also addresses a subtle but important point: governance is not bureaucracy. Effective governance makes programs faster and more decisive, not slower and more cumbersome, because it puts decisions in the hands of the right people at the right time with the right information.

    Programs that fail often do so not because of poor planning or poor execution, but because of poor governance. The board that rubber-stamps every request without scrutiny. The sponsor who is absent during critical decisions. The escalation pathway that sends every issue to the CEO because nobody else has been given authority. The phase gate review that is treated as a formality rather than a genuine evaluation. These governance failures create environments where problems compound until they become crises. Effective governance prevents that spiral.
